Document Requirements for CFO
NBI Site NBI- National Bureau Investigation ( They have Application by On line site. but seems to difficult to sign up and even if you have Application Papers you still need to line up in Hundreds People Waiting on outside.)
NSO-Nationl Statistic Office- ( Well we need to get this Papers everytime we find work,to marry,to get birth to get Passport and visa. and Some way I get Irritated to casher in NSO they are rude and no manners.)

Two (2) valid identification cards (IDs) with photograph;
Duly completed guidance and counseling form;
If married, original and photocopy of marriage contract on security paper from the National Statistics Office, or Local Civil Registry Offices; or original and photocopy of marriage contract duly authenticated by the Philippine Embassy/ Consulate (if married abroad)
Other documents as may be required by the counselors; and
Payment of P400.00 counseling fee.
Requirements for Registration
Original and photocopy of Passport;
Original and photocopy of visa
Original and photocopy of Immigrant Data Summary for US-bound emigrants (except for K visa holders);
Original and photocopy of Confirmation of Permanent Residence for Canada-bound emigrants;
Original and photocopy of Certification of Eligibility for Japan-bound emigrants;
Take note:
Passport Original and Copy ( well not only one copy make it 5 copies or more because if you wish to renew your passport they need copy also. and also save it for your self Scanning your Passport save it to your memory card or Emails.) atleast you have clear copy and also Passport copy of your partner
Not only Passport all Document you have Oo /opo/ yes Ja.. ja ..seriously
and also find a sweetest picture or wedding picture some kind of picture that you are really haverelationship. but if still long distance relationship and get a fiancee visa maybe print out some of your conversation ym or skype, fb or emails or where do you met by on line. well don't offend if they ask this
